How Update Word Document with Checkbox and Radio Button option values?

Hello, dear colleagues,

The client has asked me to make the final form identical to their paper form. I've done this type of work before with multiple forms I already have, but I realize now that in previous forms, I've never had the dilemma or the need to fill in the checkbox or radio button options previously selected on the digital form. Perhaps this solution is very simple or not, the problem is that I don't know how to do it. The ultimate goal is to try to fill in the data from a Laserfiche form into a Word document, which would then be converted into a PDF and emailed.

I'm sharing a small screenshot of the digital form and what the form looks like in the Word document I want to export to illustrate this a bit. I'm not sure if it will be enough to support my request.

I would be very grateful for any help or ideas.

I may be wrong here but I think this would be much easier to do with a PDF than a Word document. PDF form fields are easy to work with in Workflow.

Hi @████████, thanks for your response. I’m currently using a Word document because I couldn’t figure out how to add a signature directly to a PDF. Do you know if there’s a way to do that? If so, I might be able to switch and use that method instead.
