Forms action history page blank

I have selected the option to include action history with the form, but it keeps saving a blank page at the beginning of my saved form. I have tried selecting both TIFF and PDF file types.

Any suggestions?

It may the same bug as what is talked about on this thread. They recommend opening a support case to Laserfiche through your VAR. 

What version of Laserfiche Forms are you running? I can guarantee that they will advise you to upgrade to version 10.3.2 as the first order of business :)

Looks like I am having the reverse of the issue that you linked. My version is 10.3.1.552. I will look into upgrading.
