The forms have been at times storing only the Action History page from forms to the Repository.

Usually the Action History Page is attached to the front of the form when stored.  

In the repository when I open the file to look at the form that was saved, no errors listed, the form page isn't there but the metadata and the Action History page is there.

I checked to make sure there was a form set to actually be saved at that step and indeed there is.
