I'm trying to make a simple list with check box. Values are from my database.

First, I want to get all lines where "facturé" = 'non',

Next, I want to mix information with text-box like below. (for the exemple, I did it using Dreamweaver).

Idk how to mix database, checkbox, and fields together.

I need help please.

I tried with collection but I can't link my checkbox to my database.
