I have been using this form for over a year and have had no problem. This month I had 16 instances go through and only one completed the process. The other 15 are stuck in progress after the user task was completed. It seems like the action the user's took was not recorded and it is now stuck in a decision loop. I have now made a default on the decision but I do not know how to push the instance along.
