How do I set check box field to only allow one selection

I am setting up a form where I would like to limit the user to one choice at a time vs. multiple choices. Use case is Add, Change, Remove selection for new hire, position changes, and terminations. Has anyone setup a form in a similar way? Please advise, thanks!

Why not just use radio buttons instead?

Anyways, one way of doing this would be to use Javascript.

When one check box is checked, uncheck all others.

Something like this.
