quickfields not storing lookup fields

I have a quickfields session that is capturing the "Last Name" and "First Name" fields from a database lookup and populating it to the template while in Quickfields, but when it stores, the fields are blank. 

Any ideas?

What triggers the look up?  Can you send a shot of how the look up is set up?

I changed from a real time lookup in the template area to the lookup under page processing and it works 100% fine
