My client is having an issue with the android version of LF Mobile. It does not show the tab for forms like it does in the preview pictures. They do have forms setup on their server, so they are not sure if they have just missed a configuration step or something else. The first picture is their device,

Can you make sure that the LF Mobile server has a LF Forms server configured?

Building off Eric's Post,
after ensuring that you have a Laserfiche Forms Server configured, when you log in, also ensure you have selected a Forms server to log into. Like this screenshot shown here, if its "none" then you won't be logging into your Forms server via the App.
So make sure that's value is not "None" and you have your Forms server chosen like below
