How to create a form with Accept / Reject buttons without the 1st step of a submit form

We are trying to create a form that will display data form the database and place it on a form that has the accept & reject buttons on it.  We don't need the user to submit data via the form, we will have that separately.    So, how do we create a form that will display data from the data and have Accept / Reject buttons without the 1st step of a submit form?

You just need to start the process modeler with a regular Start Event (not a Message Start Event). Connect that to a User Task to provide your form and use the properties window to disable the Submit button and enable the Approve/Reject buttons.
