what could cause workflow not to run?

one of my workflows has stopped running completely.  i have ran the business process, i have ran the workflow manually, and it is still not working.  i have also restarted services and restarted the server.  what could cause this issue?

How do you determine it's not working? Are you looking at the search results in the Workflow Designer or are you just not seeing documents moving as expected if the workflow ran?
