We are using the LF scan window and have never run into this. After clicking the SCAN button, it does not scan. It does not initiate the scanner to scan. I cold-started (like the solution for the "double page feed" error) thinking that it might be reset the USB settings. Still won't scan. All setting are correct.

Are you able to scan outside of LF without LF involvement?

yes to both questions. I've not ever seen this happen in the four years we've been using it and hosting LF client scanner training on these same eight computer scanning stations.

Can you reboot the machine and login with a different windows/LF account? Does it work for a different user on the same machine?

If you believe all the settings are correct, and you're able to get into the scanner options properly then It's probably not scanner settings. Can you try and set it to scan to a specific volume and not the Parent Default?
