We had an issue yesterday where a form that was submitted on 3/25 seemingly got submitted again. The submissions each got a different submission_id (8089 and 8330) but they have the same unique_id and the forms have the exact same field values. How is this possible?

Since the possibility of two GUIDs colliding is virtually zero, we believe something funky is going on. At first we thought the user must have clicked submit twice or something, but the submissions have different dates and were made from two different IPs.

Can someone from Laserfiche explain why this may be happening? Version is 9.2.1.
