Checked out document not locked on server

A user checked out an Excel document then closed it and didn't work in it for about a half hour. Then when they went to open it from Laserfiche they noticed the check out pencil icon wasn't on the document. When they opened the document this popped up. 

 

Does anyone know why this may have occurred or have had this happen?

An administrator can force the check-in.  Did you verify if one of their admins did that?

No one force checked it in.

What version of the client and server are they on?

Client is 9.1.1.548
Server is 9.2.1.562

Is there a reason why the client is an older version? Please upgrade the client to 9.2.1 and apply SP2 for 9.2.1 to both the client machines and the server (https://support.laserfiche.com/KB/1013737
