Troubleshooting Laserfiche Forms Error 500

We have started using Laserfiche Forms 10.  It works well on one of the forms I built but the other, more complicated one is failing.  I'm able to complete the form without troubles bit I'm encountering an error when submitting it.  

Here is the relevant line out of the IIS log:

I have checked the Laserfiche Web Administration Console but it does not show any messages with the source as Laserfiche Forms.

Do you have any suggestions where to look for troubleshooting information?

The Laserfiche Web Admin Console does not cover Forms. Look in the event viewer on the Forms server under Applications and Services Logs\LFForms.
