Has anyone had problems with fillable forms?

I am trying to fill out an I-9 form with Laserfiche Workflow. It's weird because its filling out everything except the Last name and First Name. I wonder if it something wrong with the form itself. Anyone else had trouble? Here are the results from the tokens pulled.

I too have seen this as well before, the issue was caused by constraints in the PDF form itself.

Items such as a field requiring a number or other constraints that the incoming data does not adhere too such a field type (ie: string, numeric)

I finally found out after testing out other forms. My HR department actually prefers to type in the name themselves right now. So it worked out in my favor!
