User Task Terminated, but I Don't Know Why

I have 2 user tasks that after being assigned correctly to a user, terminated unexpectedly. I have no idea why and there is nothing in the error log for the terminated instance. Is there a way to re-enable it? Below is a screenshot of the instance details of one of them:

Is there anything in the event viewer on the server hosting Forms?

There are no errors pertaining to Forms.

Hi there, Is it a direct assignment, or through the team, is there boundary event on this User Task?

It is assigned to a Team with a filter associated with it. Testing the filter on that user task shows 1 team member, which is correct.

I'm not sure what you mean by a boundary event.

Is there error in the event viewer? Can you share it? Maybe you can send me the error log and the process xml (export the Team and the process) in email.

Just sent it. I did not see any errors in the application log that were in the same timeframe as the Instance details shows.

Hi Blake, Forms events log location is no longer under Windows Logs->Application, it is changed to Applications and Services Log->LFForms since 10.0.  Can you check the error log from the new location and send the error log to Abby?
