Workflow and Forms not communicating

My client keeps receiving an error message in windows application logs. The user is receiving a routing error when running a workflow with forms. I checked both the web service URL in workflow admin console and the forms server primary URL and they both seem to be correct.

Your error message says Forms is receiving error 500 from Workflow when it tries to run a service task, not the other way around. So you need to look at the Forms Configuration and check that there is a Workflow server configured in the Laserfiche tab.

Okay, I checked it and it showed this http://screencast.com/t/YPrF5RWHd which is correct. That is the name of both the LF server and Workflow server. After double checking I still recieved the same error.

Is the Workflow Web Service feature installed?

Yes, I see the web services node in the workflow admin console and it appears to be set up.
