I have two forms using the same theme. For some reason one form looks totally different and does not display the logo. Anyone have any advice as to where I need to look to solve this. I'm sure I'm overlooking something, but apart from one form having the titles above the field and the other (with the logo) having the field captions to the left of the field, I cannot find a difference.
