Exisiting workflow skips steps

I have a workflow that has been functioning perfectly until today.  I noticed in my search results window that the workflow had skipped steps.  It happened for a number of different documents and it skipped different steps.

 

 

Does anyone know what would cause this to happen?  I am running Workflow version 9.1.0.328.  I have plans to upgrade the version but was waiting for an imminent server move.

OK I did modify the workflow but I only changed a condition to add a parameter.  Something I'll have to watch out for!  Thanks for the info.

If you published an update to the workflow definition and updated running instances, it might end up looking like this. For ex, if the workflow instance was in  the Delete Entry step when it was updated with the definition that adds the Email and Assign Field activities, the new activities wouldn't have been executed.
