i am having problems when i do a lookup rule on a form. basic table with campus type and campus name. in the lookup rules i set up the available source/table, fill campus/dept type with data source value Type, then add when campus/type matches data source column Type, fill campus/dept name with data source value Name. the Type comes out but nothing comes up for Campus Name. If i do them individually data sources come out but will not link when setting up the lookup rules. any idea how to fix this issue? i attached an example of what im trying to do.

Everything in the setup itself looks correct from what I can see.
Have you tried a different browser? I know some times when I am working on a form for a while the browsers cache causes problems and simply clearing the cache or using a different browser solves the issue.

I would check the data in the database to make sure that you have a value in the name column for the campusdept value you are selecting.
database has type which includes headstart, elementary, secondary. name includes the campus's names.
