Publishing results in a blank form.

I've created a purchase order form and followed the steps located at https://www.laserfiche.com/support/webhelp/laserficheforms/9.0/en-us/forms/#CustomizingYourFormExamples.htm for Purchase Order Example. The form previews just fine, however, when publishing it, all that's displayed is a blank form as seen below.

All of the fields and theme formatting are missing. I'm able to view other published forms just fine.

Any suggestions?

I interpreted your question incorrectly. It seems that your business process has multiple forms. Please confirm that the "Message Start" event in the business process modeler is set to use the correct starting form.

That was indeed the issue, thank you very much! Once I deleted the starting form, which was blank, the purchase order form published correctly.
