Form look-up rules conflict

I have a form that is looking up a person's contact information based on their department name.  When the department is chosen the associated person's name with their phone and email are supposed to auto-fill.  It works correctly if there are multiple people associated with a department and the user picks a name from a drop-down.  If there is only one person associated with the department the name populates but not the phone and address.  It's like the form doesn't know to use the name as a look-up if the field auto-populates.

 

SQL table looks like this:

Department                      ContactName      ContactPhone            ContactEmail

Public Works                     Joe Smith            555-555-5555            person@email.com

Public Works -Roads         Nancy Jones        222-222-2222            person2@email.com

Public Works -Roads         Mel Brown           444-444-4444            person3@email.com

Hi Sandy,

What version of Forms are you using? I'm using the latest release of Forms 9.2.1.1069 and this works fine for me. I have a form set up in the same way and when I select a department that only has one person as a member, then the name, phone number, and email are automatically populated.

Version 9.2.0.853  Looks like I need to upgrade.  Can I just go over the existing installation and not have to change the version of any of the other products (server, workflow)?

Yes, you can just install Forms 9.2.1 on top of your existing Forms 9.2.0 installation.
