Saving Attachments to Repository is Not Working

I have a Forms Process that has a single task after the Start Event. That task is a save to repository task. It is currently setup as follows:

The attachment is set to save as follows:

When I submit the form, it submits successfully, but the instance is in a suspended state. With this configuration I am expecting to NOT see the submitted form, but only the attachment. Have I configured something incorrectly?

I may have found the issue, it looks like when the form "Save as" option is set to "No Form" it does not work. Once I unchecked the "Save as" checkbox, it submitted correctly.

When select "Save as No Form", it will save a document that has metadata but no actual image or electronic document component.

If you are expecting to not see this blank document, you can clear this save as option.

However, the service should not become suspended when using these settings. Can you check the event viewer if there is any error log? I guess you got access denied on the root folder.
