I'm having an issue with one specific data source losing process mappings.  The actual mappings don't get lost, but they won't show on the Data Sources screen.  The sequence of events is as follows:

• Add new Data Source 
• Add/remove Processes to allow them access to Data Source
• Processes add fine and everything works fine
• Navigate away from the Data Sources screen and come back and the Process mapping section shows that no processes are mapped, but they are mapped and continue to work
• If I go to add another process to the Data Source it will remove any ones that I don't have checked that were already there.
• This only happens with one of my Data Sources, which happens to be SQL 2014, if that has any bearing.
• I'm running Forms 9.2

I know this is a little confusing.  Below are some screenshots.
