File Upload will not save template and meta data

I have a form in Laserfiche forms that allows for a file upload. It is attached to a template with metadata mapped to the fields on the form. Once it is stored inside Laserfiche the metadata and template are no longer attached to the file? Any ideas? Occassionally it will bring the metadata/template in but more often than not it is all missing.

Are there any errors reported in the application event log on the Forms server? This issue could occur if one of the template fields is required, but there's no associated form field data. This can also occur if the form field data that's mapped to the Laserfiche field exceeds the width that's been configured.
