Direct Approval Issue in Forms 9.1

When I attempt to perform direct approval I keep receiving an email that indicates the approval has failed or already been approved.  The email text is below.

"The previous email approval (action: Approve) failed. Either task has been performed or you cannot perform approval actions."

This only happens when I approve, if I reject it works as it should.

The approval seems to work but we keep receiving these emails indicating an issue. I have searched through answers and the support page but I can't find any information on this.

Can you check whether it works if you change the label for Approve and Reject for the user task? It's wired that only approve can't work but reject works. Also please check from events log for the Forms server whether there is any detail error logged when the error "The previous email approval (action: Approve) failed. Either task has been performed or you cannot perform approval actions." occur.

I changed the Approve and Reject for the user task and it still created the email indicating the approval failed. It also is now failing when I choose Reject.

I have attached the logs from windows.
