Feature Request: Likert Field

There has been some interest in using Forms to send out surveys to staff members. For instance, when an IT Help Ticket has been closed, in the resulting email include a link to a Laserfiche Form that would take a survey about how the employee feels the case was handled.

It would be nice if Forms had a Likert field to accompany this idea, such as the one below:

An admin would be able to add additional options for the scale, change the order, and remove options.

Thanks for the request. For clarification, is there something specific that you're looking for which the radio button field is lacking?

One thing would be to have more than 4 on the same line. Also, have the radio button choice labels at the top rather than next to the radio button.

For a single row, this is possible with some CSS

but I can see a use case of having a pre-built field of this type, especially if there will be a number of survey questions. I'll file an enhancement request for it.
