Is there any way to make the comment box required? as the user can simply select a reason and skip providing a comment.

I'll assume you are referring to the advanced Audit Trail function of prompting a user for a reason when they perform a specific action. In this case, reasons can be made required, but leaving a comment in addition to a reason is optional out of the box. If your goal is to collect more information about an action, you could have an administrator create a more detailed list of reasons to present to users. In this way, the user would be required to be more specific about the reason for their action, while an administrator would not need to rely on the user to add a comment.
