"Laserfiche Forms has encountered a problem"

When submitting a form, instead of getting the "Thank you, your forms has been submitted", I am getting a message that says "Laserfiche Forms has encountered a problem. An error occurred while retrieving your form. Please try resubmitting your request later." However, the form is sent to the repository correctly with all Metadata needed. What would be causing this error to occur?

Form was submitting correctly before and it is the only Form that is submitting this way.

Version of Forms is 9.1.0.1441

I would open a support case with Laserfiche.

try clearing your cache. Might be using an old version of the same form and causing trouble on the front end.

EDIT: Also, make sure your account is logged into Forms still, just as something to confirm that you do have access to the form you are trying to submit and again, that it isn't just stored in cache

I would also highly recommend updating to Forms 9.1.1.1522.

I have cleared the Cache and upgraded to 9.1.1.1522, however I am still experiencing the same behavior.
