On the forms start page one of our customers has a Red Circle Question Mark logo instead of an image of their form. This is the area where you click Play to start a form, it normally shows a thumbnail of your form. When hovering over there is no informational message. They are asking what this means.

Hi Chad,

This occurs when a Forms process with a Message Start Event does not have a specific form selected for the start event. Selecting a form from within the Process Modeler should resolve the issue you are seeing.

Hi Chad,

Sounds like something has gone wrong with that individual form. Do you see the same thing from all workstations? (trying to rule out a browser issues with 1 specific machine)

Try upgrading to the latest version of Forms?

Failing that you could try exporting and importing the form that has the error. Also are there any messages in the windows event viewer on the Forms machine?

Maybe there are some logs somewhere which might give us some more information. Need LF to chime in on that one though as I have no idea where any logs are for Forms.

Here is a screenshot. It is not an unexpected error, just a logo of some sort someone designed.
