Workflow filling PDF with Duplicate Entries

I have a PDF form that I need to fill out and since it's a log I will be coming back to the form periodically to add more information.  Since this document is a PDF and will be used on an iPad which can't store the document with check-in, check-out I have come up with a workflow.  I will have the user fill in the template fields (which are multi-value fields) and every time the document is altered the workflow is supposed to append the values.  It works fine except that when it appends the first time it inserts the same data in three rows.  When I make a second entry it changes the second row correctly but also puts it in the third row and so on.  I've attached the form and a screen shot of my workflow.  If you tell me how I can upload the actual workflow.

 

If I can make this work that would be great or I'm open to other suggestions.  I have decided not to use a Laserfiche form to do this since different people will need to come back to a form multiple times and I think it will be too confusing.

I'm not seeing the behavior you're describing. Could you open a support case so we can take a closer look, please?

I was able to figure this out by using the Table tab on the PDF fill object.  Didn't know this existed before.  Thanks for your help.
