The City has standardized on IE 8 for the web browser (a long time ago, I know, don't say it please).  Whenever I send a notification to a forms participant and they click either Approve or Reject they get a screen that looks like the attached.  Is there an incompatibility between IE 8 and Forms 9.1.1?  Do we need to move to the current version of IE?
