Some fields unavailable in a PDF when named the same

I have a fallible PDF that has the same field at the top of each page called "Name." Only the first page where this field appears is able to be selected in  Workflow to be filled. The others do not appear as a field in the left pane and do not appear in the list of fields in the PDF on the right pane.

 

I have another field called "Mother" that appears multiple times (on the same page and on different pages) and Workflow sees these fields as independent and does  not have a problem filling in these fields using a multi-value token.

 

Why is the behavior inconsistent?

Could you attach a sample PDF or have your reseller open a case with Tech Support and send one to them, please?
