We are using forms to save to the repository that has info that is pulled from a database, we have been hitting a few snags and we sometimes receive an error image instead of the completed form, can anybody make sense of these error images?

It appears that the Forms database needs to be tuned because it's taking longer than 60 seconds to run a query and it shouldn't be taking that long.
Please update statistics and rebuild indexes in the Forms database. If there's still an issue after doing so, then create a case with Laserfiche Support so the issue can be looked into further.
