Municipal Complaint Process

I have been asked to create a Complaint Form and Process (likely initiated off the County website) and it appears we're starting from scratch.  Is there a Municipality out there that has already created this, or a similar, process that would be willing to share what they have done?

UPDATE:  I started this process on Laserfiche Forms.  The main form for residents to use was complete but not yet activated on the website.  Once a resident fills out the form and hits Submit, a subform would go to the people responsible to respond.  It was emailing the right people, but timers were not working right before the project was put on hold.  I wanted it to have timers so that if one person didn't respond within one hour, the email would be sent out to a secondary choice, then a third choice.  

The idea was an initial supervisor would get the first email, then he/she would assign an individual to investigate.  They would get the email on their mobile phone, go to the site, take photos from the site and upload to Laserfiche for documentation, take care of the problem and/or report back to the supervisor via the Form submission process.  Once done, all would be recorded in a report in Laserfiche.

This project was put on hold before it was finished, as we learned that Bellamy was currently creating their own version of it.  We use Bellamy, so we will wait and check out their process once they've completed it, before determining if we want to continue with our own.

The actual form as seen on the website, which uses dynamic fields to populate questions for clarification depending on which type of service request they choose (the radio buttons):

I am also interested in this - as a way to show value of Rio - a demo of something like it. Doesn't have to necessarily be exactly a municipal complaint tracking system, but this is a big pain point in our organization. Anyone doing it or something like it?
