"Forms Configuration Failed to Load"

Sorry, newbie alert.....

 

I just installed Forms 9.1.1.  When I try to configure forms, I get a pop-up indicating, "Forms Configuration Failed to Load....... Unable to connect to the Forms Routing Service".  In the services listing, I can see the Laserfiche Forms Routing Service is set to Automatic.

 

I can't seem to find any further information on ANSWERS regarding this condition.  Nothing appears in the application event log.

 

Can anyone offer next steps to resolve this?

This may sound like a silly question but can you confirm that the Laserfiche Forms Routing Service is started?

It is not.  When I try to start it manually, i get a message say that it started and stopped, and that sometimes this is normal operation.

Can you post the details of the "Application Error" listed in your Event viewer screenshot?

Hi Dave,

 

What Service Account did you use? Does that account have access to Forms Server database?

The service account used for the Forms Application Pool and Routing service is an AD account with full access to the repository.

 

The SQL database that has been created, has been setup to use just SQL server authentication, (ie. a different, non-AD account).

 

 

I am trying to configure Laserfiche forms, (page 19 in the guide).  In the configuration interface, I believe that I need to configure how Forms will connect to the database.  The problem is that get the above error, when I try to open the Forms Configuration tool.
