LF 9.1 and Outlook 2013 Add-In Not Working

If I enable the Laserfiche Outlook 2013 Add-In, Outlook will not open.  If I disable the Add-in, then I have no integration with Laserfiche.  Is there something I'm missing?  I've tried uninstalling and reinstalling both Laserfiche and Office.

Please contact your Laserfiche reseller to open a support case. The Laserfiche 9.1 Office Plugin is compatible with Outlook 2013. Perhaps there is another third party add-in that's installed and conflicting with the Laserfiche add-in.
