Forms Title Text Vertical Center Align

Can someone tell me how I can get the text of a form title to be centrally vertically-aligned with the associated form logo, i.e. so that the form title text is

I have tied all combinations of CSS to try to get the result I want, all without success.

Please see the attached image for what I am currently getting and what I want to achieve.

It may be easier to just select the Laserfiche theme and then customize it with the colors and logo you need and save it as it's own theme. The Laserfiche theme already does what you are asking for.
