Creating entries that are shortcuts or links to pages in a document stores elsewhere

Here's my scenario:

 

We have a folder called Outside Medical Records, and we have a folder called lab results.  The Outside Medical Records contains entries that are complete histories of for a patient at a particular institution.

 

Our folks go through this history, and find where the lab tests reside (let's say that's page 46-48).  

 

Task #1:  I'd like to keep the original document intact and leave it where it is, but I'd like to create an entry (preferably a shortcut or a link, and not a copy of the pages) in the Lab Results folder, assigned with metadata, like date of service, that will reside within the Lab Results folder.

 

Task #2:  If possible, I'd like to remove this shortcut or link once the service date for this particular document or page exceeds one year.

 

Any thoughts to an approach?

I've never found a way to do this. I've asked about it before, but I don't mind asking again.

 

It would be nice to have "bookmarks" within a document, similar to how they work in a PDF, but be able to reference those bookmarks in other documents, and using shortcuts. This would allow cross referencing, which is a feature that's been asked for by both the auditors and detectives that I work with. Links are good for some things, but they only get you to the top of the document.
