The best place to start is the Event Viewer on the Forms Server, since that is where all errors (if any) will be recorded. If you don't find anything there, I'd suggest attaching your business process so we can take a closer look at it.

Looks like I am receiving an Oracle data error.

We were doing a database lookup to an Oracle database, but removed the lookups so the Oracle database should not be accessed at all.

I think I have narrowed down the problem. It looks like it is related to adding information in the Comment fields. If a user does not add any comments, it saves to the repository correctly. When someone adds a comment it sits at the Save to Repository step. When it saves to the repository we have it including the action history.

I unchecked the Include the Action History option and when making comments it saves to Laserfiche correctly. Is this a bug?

Just yesterday I opened up a Support case for this exact same issue with another client.  I feel that this is a bug because if you want to include Action History and someone makes a comment while you have that box checked, then your document never Saves to the your Repository.  So I feel that this is an actual bug within Forms.
