User Settings Not Filled In?

I looked at the user settings for my login today and noticed that my Username was correct, but there was no information for my Name or Email. Reading the help file it states that it should pull the name and email address from the Windows account. Looking at my Windows Account in Active Directory, AD shows my display name and email address, but it is not being brought over to Forms. Any ideas of how to troubleshoot this?

I got it figured out. The user that was set on the Configuration for Repository was not selected as a named user.

On the Forms Configuration page, did you fill in the Active Directory domain field with the name of your domain controller?

Yes, Active Directory domain and Windows domain are both filled in.
