Where does the value for the "Form Name" column when looking at Results populate from? In all of my Forms that value is empty.

The "Form Name" column displays the name of the last submitted form in the business process, for all ACTIVE BPs. Once the BP has been completed or terminated, the "Form Name" column is cleared.
