we have a form that has a series of yes/no questions. If yes is selected, we have a description box appear to fill out the information. The description box is also required once it appears. If no is selected, then the box does not appear. The issue we are running into is that a form can be filled out and submitted with no problems but, even if no is selected, the approver still is told that the description box needs to be filled, or they cannot approve it. How can we fix this?

I'm guessing that you are not Ignoring data when hidden in your field rule. If you have it set to Save data when hidden, you'll get the behavior you are seeing.

I just double checked and it's set to ignore data when hidden

I think I understand a little more, so this is after it's been submitted. So we're now at an approver task is when you're seeing this?
Without seeing your form and process, what you can do is add another field rule to validate that field as Optional when you are at that process step or process stage.
