For Each Value loop is creating multiple annotations

I have a simple business process that pulls values from a multi-value field and puts each value into a sticky note, then clears the values from the multi-value field. After changing it to a workflow, it creates multiple sticky notes for each of the values. What am I missing?

How does the workflow start? Are you sure it's not multiple instances running on the same document?

Do you need them in separate stickies though? Or would one sticky note with the list of values be sufficient? If the latter, you don't need For Each Value, just use token formatting on the multi-value token generated by Retrieve Field Values to separate values with a comma or newline in the Add Sticky Note activity.
