Question

I know we can automatically fill out a form from a table via workflow. In the table, how does the data for a checkbox need to be formatted?

In this case, it basically going to be a variable that is either checked or not checked. Those are the only options.

Also, what would that look like for a checkbox that had multiple values?

We match based on value, if the lookup or designer defined choices for a checkbox/radio/dropdown match the value it should remain selected. This should work for fields inside/outside a table or collection. Just make sure to use multi-valued tokens to fill tables/collections.
