Has anyone seen this behavior in Laserfiche Forms Monitor?

In the Monitor tab, most tasks show the expected green checkmark and "Completed" status with full audit details (Action By, Action, Comments, IP).  However, occasionally we see a task where:

• The workflow has clearly moved past the step
• The task shows both a start and finish date
• The status still says Assigned or Reassigned
• The icon is a black box instead of a green check
• No Action By / Action / Comments / IP are recorded

Very intermittent.  Mostly on one new process but have seen it on others.  The new process is heavily customized on the form side (JS/CSS), but no custom submit/approve interception.  Workflow logic is standard routing.  Some of the tasks did have due dates and I thought they may have had something to do with it, so I removed but didn't resolve.

Is this a known bug?  Has anyone ran into this before?  Appreciate any insight - this is a first for me.
