Query Forms submission field values with labels

I want to query the forms database and get the submitted field values along with the field labels but when the field is a subfield within an Address field, I am not able to return a label.

Is there an easy way to get the labels for the individual subfields of an address field?

Hi Bert, the labels for subfields are stored in [dbo].[members], you can use [member_id] from table [dbo].[cf_bp_dataset] to get them.
