I need to replicate this format in a collection or table with field labels (rows) on the left but my attempts have been fruitless. The labels end up on top of the field, instead of on the left.

I only need this format for a single collection and not the whole form (should be labels on top of field for the rest of form).

How do I accomplish this with Laserfiche 12 forms in the modern designer?
