delete outstanding forms from a deleted account

I have a user that got married.  I added a new login for her with her married name in ad.  For some reason, Laserfiche didn't like it (could have been ad, not sure).  I deleted her account in ad which also removed it from Laserfiche.  I recreated her account in ad and Laserfiche and everything seems to be working ok now.  It appears that she is still getting a reminder email about a form she needs to finish under her deleted account.  If I look in forms - monitor I don't see the form in question.  Is there a way to find it in SQL and delete it?

I use this script to pull instances assigned to users:

You can search by their old email or username to find the instance that is still assigned if it is active.
