Incident Reporting Form/ Near Miss Use Case

Our agency has a Safety Incident Reporting Form, and I hope to use Laserfiche to streamline and automate the process.

The form is to be completed by an employee after a work incident or near miss occurs

  1. The employee fills out the employee portion, signs, dates, and forwards it to their immediate supervisor
  2. The immediate supervisor then fills out their portion, signs, dates the form, and forwards the form to the department manager
  3. The department manager then signs and forwards the form to Safety
  4. Safety Staff conducts a review and assessment, signs, and files the form

 

*Only the employee, immediate supervisor, department manager, and Safety staff should have access to the form once filled out by the reporting employee.

Is this process achievable in LF? Thank you!
